We welcome all new patients to visit this page to learn about what to expect on your first appointment. Our staff are always happy to chat with you if you have any questions.

Setting up an appointment doesn't need to be a daunting task. Follow this guide to booking your appointment with us or call us if you need any assistance. We are more than happy to help you with any and all questions you might have.

How to Book Appointments

For your convenience, we are now offering online booking for Physiotherapy, Chiropractic, and Massage Therapy appointments. Booking online is easy and only takes a few minutes.

You can call us at 905 669 1221 to book an appointment. Our front desk staff will be happy to answer any questions you may have regarding treatment, billing, and our clinic.

Write us an email detailing your problem and any questions you may have regarding our service. You may request an appointment time via email, and we will confirm as soon as possible.

Getting ready for your visit

Fill out your intake forms
If you are able, you may either:
i. Fill and sign the digital "Informed Consent Forms" and "My Medical History" forms in the patient portal
OR
ii. Print the following completed forms to your initial appointment.

iii. If you are unable to do either 1 or 2, please come 10 minutes early for your appointment and our front desk will assist you.

Bring a change of clothes
Wear appropriate clothing for the area that you would like to have assessed and/or treated. Make sure you are comfortable and that the clothing is not restrictive. (i.e. if you are having your knees looked at please wear or bring along a pair of shorts). We have private areas for changing in our facility.

Team Screening
Our team is fully vaccinated, and we routinely perform health screens on our staff. Our treatment areas are sanitized after each patient treatment. All high traffic surface are wiped down frequently, and our staff wash hands before and after patient interaction.

Bring a face mask to your appointment (Optional)
Please bring in a cloth face mask to wear at your discretion during your appointment. Our staff may wear masks as this is recommended by health professionals in higher risk settings.

Checking-In

Wait until your scheduled time
If you arrive early, kindly waiting outside or in the parking lot until you are called in for your scheduled appointment. We will give you a phone call when you are ready to come in for your appointment.

Limit Guests
Please do NOT bring guests into the clinic unless it is necessary (ie. translator, personal support worker, substitute decision maker, parent/guardian, etc.)

Sanitize Hands
Upon entering our clinic please sanitize your hands with our complimentary hand sanitizer provided or use our washrooms to wash your hands.

Temperature Check
Our front desk staff will check your temperature with a touchless thermometer. If your temperature exceeds 37.2 C, you will be asked to return home until you feel better.

After You Visit

Thorough Sanitization Procedures
Our staff ensures that all high traffic areas are wiped down including beds, machines, and exercise equipment. These are in place so that each patient feels comfortable with our procedures

At Your First Visit

What to expect during one of our consultations

All of our therapists will perform routine and structured approaches to their practice. Our therapists are all highly educated and have years of combined experience to deliver high quality, effective care. Click here to see our qualified team of health care experts!

Sign in.

When you arrive please check in with our front desk. If it is your first time you will be asked to fill out your intake forms if you haven't already.

Detailed History Taking.

It is routine for our therapists to have a detailed interview regarding your medical and/or injury history. This helps us better understand your problem and create tailored solutions.

Objective Physical Assessment.

We use a variety of clinical tests to get to the bottom of your complaint. Often we will take measurements to track your progress.

Get Treatment.

After completing the assessment, your therapist will determine the most appropriate management for your injury and recommend a treatment plan. Our care coordinators at the front desk will help set up your treatment schedule.

Sign out.

After treatment, find your way back to the front desk where our care coordinators will guide you through any questions you may have with billing, insurance, and setting up your appointment schedule according to your individualized treatment plan.

What else to bring?

• If you have a referral from your doctor please bring this along. You do not require a referral to come see us.

• If you have imaging studies/reports done (X-ray, Ultrasound, CT, MRI, etc.) please bring along a copy for our provider.

• Bring along your insurance information. If you have been involved in a motor vehicle accident please bring along your insurance company’s name, policy number, claim number, as well as the adjuster’s name and telephone number so we can bill your insurance provider.

• Bring any specific equipment that may be relevant to your problematic area. This may include walking aids, braces, specific footwear, insoles, and other assistive devices.
